On or before January 1, 2012, and annually thereafter, the commission shall submit a report to the Legislature. The report shall include all of the following:

(a) The number of participants in the program.

(b) The locations where the participants practice or attend school.

(c) The amount expended for the program's operation and the amount collected in donations for the program.
